What is Isturisa (osilodrostat) for?

How does Isturisa (osilodrostat) work?

Osilodrostat stops the body from making cortisol. It blocks the enzyme 11-beta-hydroxylase, which makes cortisol.

For Cushing’s disease, surgery to remove the tumor works best. But sometimes, surgery can’t remove the tumor fully. In these cases, osilodrostat can help.

Where has Isturisa (osilodrostat) been approved?

Isturisa (osilodrostat) has been approved by:

  • The European Medicines Agency (EMA), Europe, on January 9, 2020 for Cushing’s syndrome.
  • The Food and Drug Administration (FDA), USA, on March 6, 2020 for Cushing’s disease in adults who can’t have surgery or have had surgery but still have the disease.

It also got Orphan Drug Designation for rare diseases.

It’s the first FDA-approved medicine to lower cortisol by blocking 11-beta-hydroxylase.

How is Isturisa (osilodrostat) taken?

The usual dose is:

  • Start with 2 tablets of 1 mg taken orally two times a day, with or without food.
  • Gradually increase the dose by 1 to 2 mg twice daily. Check cortisol levels every 1-2 weeks until symptoms improve. Don’t increase the dose more than once every 1-2 weeks.
  • If a patient can handle 10 mg twice daily and cortisol levels are high, increase the dose by 5 mg twice daily every 2 weeks. Check cortisol levels every 1-2 weeks until symptoms improve.
  • The right dose of Isturisa is found by adjusting based on cortisol levels and symptoms.
  • In studies, doses ranged from 2 mg to 7 mg twice daily. The highest dose is 30 mg twice daily.
  • After finding the right dose, check cortisol levels every 1-2 months or as needed.

Before starting Isturisa, check blood levels of potassium and magnesium. Also, do an ECG to check heart function before treatment, within a week, and then as needed.

Lower the dose or stop treatment if cortisol levels are too low or if symptoms suggest too little cortisol.
